—-Followup Visit—-

  • Our team will schedule a follow up visit for you at your first visit. The time and need for follow up varies.  As labs consults and X-RAY's come in Dr gray will review these and we may schedule a phone or in office visit depending upon results.   We will also send a letter in the mail to your preferred address informing you of the next step(s).

  • We have an on-patient secure portal.  If you have an email address we will use the  portal for reminders and results.  Dr Gray  prefers to discuss results in person so we can clear up any questions you or your family might have. 

  • Tele health appointments are scheduled after office hours.  We attempt video but if you don't have access to this technology a phone call is great.

  • Not all follow up information can occur by telephone.  We will let you know if this is the case.

    —-Emergencies—-

  • If you have an emergency please call 911.  Our office team will screen all messages. Emergency messages will be returned within 48 hours our earlier. Please use the emergency line.  

  • Most of all relax knowing our team has your best interest in mind. 

  • ER, urgent care, specialist reports.

  • Kindly remember to bring any paper work you received into our office at your next visit
    To ensure privacy, federal law dictates that any records from you must be received by hand delivery or fax. 

  • Importably Dr gray will provide you with a written visit summary.  This is our agreement on the next steps toward maintaining your health care.

—-New Patients—-

  • If you've never been to our office before you are in for a treat. Relax in our quiet waiting room. Our team will make a computer chart for you

  • Please bring your current insurance card(s)

  • Be ready to take a picture so we can see your smiling face.

  • Our team will ask your consent to access your pharmacy medications.
    Importantly please bring a copy of any medication you are allergic to.  
    If you have specialists it is best if you can bring their office card so we can make sure to continue your care.

  • Please have at least one hour for the visit so we can make sure we log all your medications, specialists, hospital visits family and surgical history accurately.
    If you have had laboratory work or X-RAY's done we will research your results if you remember where they were done. If we cant access these results we may ask you for your consent to get the old records. 

  • If you are in a managed care plan we may need to get approval for certain medication refills specialist follow up or old x-rays results.  This may not be able to be accomplished on your first visit but we will do our best.  
    Please help us to help you.  Medications vendor's changed and may not be made in the exact size or tablet that you had before.
